Code of Conduct - Brit Kehillah
I hereby grant permission for my child to be a member of the Youth Group and to participate in activities. This will serve to release the Youth Group and Temple Beth Miriam, from liability in case of accident or injury resulting from all causes in connection with such membership events. In granting permission and release, I specifically recognize that my child may be transported to events by private vehicles operated by volunteers not as agents, employees, or representatives. I also recognize that from time to time, I may be called upon to provide transportation to certain events as a volunteer. In such regard, I release Beth Miriam Youth Group, its officers and its employees from any liability, which may arise as a result of such transportation whether or not it is organized by the Youth Group. Both my child and I have read the Youth Group Brit Kehillah – Code of Conduct, which is posted on the Youth Group Website. My child recognizes that he or she will be held accountable for their behavior throughout the events.
The participant has my permission to engage in all prescribed activities except as noted above. In the event that I cannot be reached in an emergency, I hereby authorize the physician selected by the Youth Director, or Temple Beth Miriam, its personnel employees, representatives, and event personnel to properly treat my child as they see fit.
